Bridges to Care provides personalized home care support during that important transition. Our team can help review discharge instructions, determine what assistance may be needed at home, establish a care plan, and provide the additional day-to-day support needed to make the transition safer and easier.   • Can you help after a hospital or rehabilitation stay?

    Yes. Our Bridges to Care program is specifically designed to support the transition from a hospital, skilled-care facility, or rehabilitation setting back home.

  • Do you provide transportation?

    Transportation can be incorporated into home care services for current Bridgewater clients, including transportation to appointments, shopping, and other outings.

  • How do family members know what's happening during visits?

    Bridgewater offers an Online Family Room that helps authorized family members stay informed about their loved one's care. The current program also uses GPS-supported clock-in monitoring.

  • What if we're no longer sure staying at home is the right option?

    Home care is Bridgewater's primary focus, but we can also help families explore senior living options when remaining at home is no longer the right fit.
